CLIENT CONFIDENTIALITY
Individuals who enroll or who have been enrolled in the Heartland Recovery program are protected by our client confidentiality policy. Without a client code, given to the client and approved contacts upon intake, employees may not confirm or deny an individual’s enrollment. Individuals or agencies requesting program-related client records must complete an Authorization of Release form which must be signed by the client and notarized. Heartland Recovery will honor subpoenas served for records as applicable by law.
Emergency contacts listed on a client’s application may be contacted due to circumstances including but not limited to:
- The client requires hospitalization or medical attention
- The client becomes a potential threat to himself/herself or others
- The client leaves against recommendation
